Bệnh lao phổi đã có mặt trên trái đất từ thời rất xa xưa. Ngày nay mặc dù bệnh lao phổi  đã được  chữa dứt hẳn nhưng nó vẫn tồn tại  và tiếp tục hành tại một số quốc gia đang phát triển nơi mà sự  nghèo khó , suy dinh dưỡng và tình trạng thiếu vệ sinh  là môi trường lý tưởng  cho bệnh lao phổi nẩy nở. Tại Hoa Kỳ tỷ lệ lao phổi đã giảm đi rất nhiều  nhưng với làn sóng di dân, vi trùng lao phổi vẫn tiếp tục xâm nhập, vì thế bệnh lao phổi vẫn còn là mối quan tâm trên đất nước này.

Bệnh lao phổi là gì?

Lao phổi là một loại bệnh truyền nhiễm gây ra bởi vi trùng  Mycobacterium tuberculosis (hình 1).

Trong cộng đồng VN khi nói đến bệnh lao người ta chỉ nghĩ đến lao phổi, thực ra ngoài phổi, vi trùng Mycobacterium tuberculosis có thể sinh bệnh tại nhiều cơ quan khác, vì thế ngoài lao phổi còn có  lao hạch, lao xương, lao màng óc, lao ruột , lao gan v.v

Bệnh lao phổi được truyền đi như thế nào? Vi trùng lao phổi thường được chứa trong các hạt nhỏ li ti. Các hạt này được phóng ra ngoài không khí từ những người bị lao phổi khi họ nói , hắt hơi, ho hay ngay cả khi hát nữa. Người ta nhiễm bệnh lao phổi khi hít phải những  hạt chứa vi trùng  này. Sự nhiễm bệnh tuỳ thuộc vào các yếu tố sau:

  • Số lượng  và nồng độ của vi trùng  trong không khí.
  • Thời gian tiếp xúc với vi trùng
  • Và nhất là khả năng đề kháng của cơ thể: những người có sức đề kháng yếu như mắc bệnh HIV, tiểu đường , ung thư, suy dinh dưỡng .. rất dễ mắc bệnh lao phổi

Vi trùng lao phổi sinh bệnh như thế nào?

Vì hạt chứa vi trùng lao phổi rất nhỏ nên khi được hít vào phổi nó có thể được đưa đến  vùng sâu nhất  của phổi là phế bào (alveoles). Tại đây trong cơ thể của những người nhiễm bệnh  lần đầu (primary tuberculosis – lao nguyên  phát ) vi trùng lao phổi sẽ bị tấn công  bởi các thực bào ( alveolar macrophages) . Trong cuộc trạm chán này một số vi trùng  sẽ bị tiêu diệt , số còn lại sẽ từ từ sinh sôi nảy nở ngay trong các thực bào . Khi số lượng đủ lớn (10,000 tới 100,000) nó sẽ kích thích  một số các tế bào trong cơ thể và biến  các tế bào này  thành một lực lượng hùng hậu  có khả năng tiêu diệt  các vi trùng lao phổi. Hiện tượng này  gọi là miễn nhiễm do tế bào (cell- mediated immunity) và được phát hiện bằng phản ứng da. Cùng với thực bào (macrophages), các tế bào này quây kín các vi trùng lao phổi trong các hạt (tubercle) để ngăn chận  sự phát triển  của căn bệnh .. Trong các hạt này, một số vi trùng  sẽ chết , số còn lại  có thể sống âm ỉ trong nhiều năm. Khi cơ thề suy yếu các vi trùng này sẽ hoạt động  trở lại gây ra  bệnh lao thứ phát  (secondary tuberculosis, hay reactivation  tuberculosis). Trong bệnh lao thứ phát  vi trùng gây bệnh chính là vi trùng  đã xâm nhập  cơ thể lần đầu tiên và nằm ẩn nhiều năm trong cơ thể . Trường hợp này trái với bệnh lao tái nhiễm (reinfection) trong đó vi trùng sinh bệnh  là một vi trùng  mới từ ngoài xâm nhập  vào. Sự phân biệt này rất quan trọng về  phương diện dịch học vì  khi có sự xuất hiện  của một loại vi trùng  mới người ta phải cố gắng tìm ra nguồn gốc của nó để tiêu diệt  trước khi bùng nổ thành một trận dịch (epidemic). May mắn là hiện giờ người ta có thể phân biệt được hai trường hợp này nhờ phương pháp  khảo sát yếu tố di truyền của các vi trùng (genotyping of cultured organism)

Làm sao biết bị bệnh lao phổi:

Trong giai đoạn đầu tiên triệu chứng rất lờ mờ như hâm hấp sốt, ho khan, mệt mỏi, ăn mất ngon, sút cân, rã mồ hôi ban đêm.. Ðau bả vai có thể xảy ra nhưng đây không phải là triệu chứng đặc biệt  của bệnh lao phổi như nhiều người đã nghĩ. Khi bệnh đã tiến triển bệnh nhân có thể ho ra máu, suy nhược khó thở.

 Vì triệu chứng  nhiều khi không rõ rệt nhất là khi bệnh nhân  còn mang thêm các loại bệnh khác như  HIV, suy thận , tiểu đường, ung thư… nên việc định bệnh rất dễ sai sót. Cách tốt nhất là chúng ta   phải đề cao cảnh giác khi thấy các triệu chứng  trên để từ đó có thể khám phá bệnh lao phổi một cách sớm sủa bằng các phương pháp  thích hợp.

Làm sao định bệnh lao phổi:

Các phương pháp sau đây được dùng để định bệnh lao phổi

1. Hình quang tuyến (hình 2): Trong bệnh lao nguyên phát (primary tuberculosis), người ta thường  thấy các vết  nám (infiltrate) ở phần giữa hay phần dưới phổi cúng với hạch lao tại vùng tế  (hilar adenophathy) cùng bên.

Trong bệnh lao thứ phát (secondary  infection / reactivation tuberculosis) người ta thường thấy các hốc nhỏ (cavities) ở phần trên của phổi. Ngoài ra  người  ta có thể thấy các loại hạt (nodules) nước trong màng phổi.. Vì những hình này có thể thấy ở các bệnh khác nữa nên không thể  dựa hình vào quang tuyến  để xác định bệnh lao phổi được.

2. Phản ứng da: (Tuberculin skin test – TST) (hình 3) Ðể thực hiện  thử nghiệm  này người ta chích dưới da chất tuberculin PPD (purified protein derivative) là một thành phần của  vi trùng lao phổi. Ở những người đã nhiễm bệnh lao phổi  phần da chung quanh chỗ chích sẽ dày lên  (induration)  và người ta dựa vào kích thước phần da dầy nầy để xác định  kết quả. Kết quả này không bị ảnh hưởng bởi thuốc chích ngừa lao (BGG) trước đó. Ðược coi là dương tính (positive) các trường hợp sau:

  • bằng hay lớn hơn 10mm đối với người bình thường
  • bằng hay lớn hơn đối với nguời  mang bệnh HIV, mới tiếp xúc với người  bị bệnh lao phổi có hình quang tuye*n nghi là do lao và những người có sức đề kháng ye*u (tiểu đường , chích ma túy, suy thận nặng, uo*ng thuo*c steroid…)

Người ta không thể dựa vào phản ứng  da để xác định bệnh lao phổi vì các  lý do sau:

  • Có một số trường hợp  bị bệnh lao phổi mà phản ứng da vẫn bình thường  (false negative) như ở những người  mang bệnh HIV, uống steroid, nghiền rượu..
  • Ngược lại có trường hợp không bị lao phổi mà phản ứng da vẫn dương tính  (false positive)
  • Ngay cả trường hợp bị lao phổi ø phản ứng da cũng không phân biệt được những người nhiễm lao (nhưng không bị bệnh) với những người đang bị bệnh..
  • Mặc dầu với những giới hạn trên phản ứng da vẫn rất hữu ích trong việc truy tầm bệnh lao phổi.

3. Thử đàm: đây là phương pháp định bệnh chính xác nhất. Mẫu đàm thường  được lấy vào sáng sớm trong 3 ngày liền. Nếu bệnh nhân không khạc đàm được người ta có thể làm long đàm bằng một dung dịch  muối ưu trương  (hypertonic saline). Dung dịch này được xịt vào họng dưới dạng các hạt nhỏ li ti (aerosolized). Ðàm được xét nghiệm bằng các phương pháp sau:

  • Phương pháp nhuộm: (Acid fast bacilli – AFB smear), bằng một phương pháp nhuộm đặc biệt người ta có thể nhận diện vi trùng lao phổi dưới kính hiển vi. Phương pháp này nhanh chóng nhưng không hoàn toàn chính xác vì có thể  nhầm lẫn với một số vi trùng khác.
  • Phương pháp cấy vi trùng (culture), đây là phương pháp chính xác và quan trọng nhất. Chính xác vì nó có thể xác định vi trùng lao phổi  và quan trọng vì nó là phương tiện duy nhất để thực hiện  thử nghiệm hiệu quả của các thuốc chống lao (drug susceptibility test). Thử nghiệm này rất cần thiết  trong việc điều trị các trường hợp lao kháng thuốc.
  • Ðiều bất tiện của phương pháp này là  thời gian chờ đợi rất lâu (4-8 tuần).
  • Gần đây người ta có thể  trực tiếp khám phá vi trùng lao từ  mẫu đàm bằng phương pháp sinh học  , nucleic acid amplification  techniques (Amplicor, Genprobe). Phương pháp này  nhanh chóng , thuận tiện nhưng vi khó khăn kỹ thuật và tốn kém nên vẫn chưa được phổ biến.

4. Hút nước trong  bao tử: trường hợp không  thể lấy đàm được thì người ta  có thể  hút nước trong bao tử để  khảo sát . Phương pháp này ít được thực hiện .

5. Soi phổi: bằng một dụng cụ quang học người ta có thể trực tiếp nhìn rõ cấu trức của phổi. Khi soi thấy vùng  nghi ngờ người ta có thể lấy một mẩu thịt (biopsy – sinh thiết) hay hút các nước rửa (lavage) vùng này để xét nghiệm vi trùng lao. Vì có thể  có biến chứng nguy hiểm  và tốn kém nên phương pháp này chỉ được dùng  trong một số trường hợp đặc biệt

Bệnh lao phổi tiến triển ra sao?

Ða số các trường hợp lao nguyên phát (primary tuberculosis)  đều khỏi mà không cần chữa trị . Chỉ có chừng 10% về sau có thể  gây ra  bệnh  lao thứ phát (reactivation tuberculosis). Tỷ lệ này giảm đi  nhiều nhờ uống thuốc phòng ngừa . Trong các bệnh lao thứ phát nếu không được chữa trị vi trùng lao phổi sẽ ăn mòn  các hạt (tubercules) để biến thành các hốc nhỏ (cavities) . Bệnh lan dần trong phổi dẫn đến tình trạng ho ra máu, khó thở, suy nhược, rồi tử vong. Một số vi trùng  truyền qua đường máu gây ra bệnh lao hạt kê (military tuberculosis) rất nguy hiểm.

Nếu chữa trị sớm  thì hầu hết sẽ lành hẳn, để lại các vết hoá vôi (calcified) hay sọ (fibrosis) trên hình quang tuyến.

 Bệnh LAO PHỔI  được chữa trị ra sao?

A. Các thuốc chống lao:

1. Các thứ thuốc chính:

  • Isoniazid (INH)   có thể có biến chứng gan, thần kinh.  Thuốc này thường được phối hợp  với sinh tố B6 đề phòng  ngừa biến chứng thần kinh .
  • Rifampin  có thể gây tổn thương gan, làm giảm tiểu cầu (platelets). Pyrazinamide (PZA) gây ra biến chứng gan, bệnh thống phong (do lượng uric acid tăng cao)
  • Ethambutol: có thể làm mờ mắt hay ảnh hưởng đến việc  phân biệt màu sắc.
  • Streptomycin: thường hay làm ù tai hay chóng mặt suy thận..
  • Các thứ thuốc hỗn hợp  không được dùng cho trẻ em dưới 15 tuổi
  • Rifamate: INH + Rifampin
  • Rifater: INH + Rifampin +PZA

2. Các thứ thuốc khác : Caprceomycin, Amikacin /Kanamycin, Levofloxacin, PAS, Ethionamide, Cycloserine. Các loại thuốc  này thường được dùng  trong trường hợp  bệnh lao kháng thuốc (drug -resistant pulmonary tuberculosis).

B. Ðiều trị:

1. Phòng ngừa: các trường hợp  sau đây được phòng ngừa  bằng INH hay RIF từ 4 -9 tháng

  • Mới tiếp xúc với người bệnh lao phổi, dù phản ứng da bình thường.
  • Nhiễm lao nhưng không bị bệnh lao (phản ứng da có ke*t quả bị nhưng thử đàm và hình quang tuye*n bình thường. Trường hợp này áp dụng cho những người  dưới 35 tuổi , bị bệnh HIV , sức đề kháng ye*u…)
  • Ðã từng bị bệnh lao phổi nhưng hiện nay, không tái phát (Phản ứng da có kết quả bị, có vết sẹo  trên hình quang tuyến nhưng thử đàm bình thường không có vi trùng lao.

2. Bệnh lao đang tiến triển (active tuberculosis) thường được chữa trị bằng 4 thứ thuốc  INH, Rifampin, PZA, Ethambutol trong 9 tháng.

3. Bệnh lao kháng thuốc  (Drug – resistant TB) trong trường hợp  naỳ sự điều trị phải dựa  trên kết quả của drug susceptibility testing (thử nghiệm  hiệu quả của thuốc )

Bệnh lao phổi có cần nằm nhà thương hay không ? Với phương pháp trị liệu hiện nay hầu hết bệnh nhân  có thể được chữa trị tại gia với điều kiện  phương pháp vệ sinh  phải được triệt để tôn trọng . Việc nhập viện chỉ cần thiết  trong một vài trường hợp  đặc biệt như bệnh nhân không chịu uống thuốc , bệnh nhân vô gia cư  (homeless), hay ở cùng nhà với các trẻ nhỏ dưới 1 tuổi  hay với các người có sức đề kháng  yếu (HIV, uống thuốc steroid, ghép cơ quan..)

 Lao phổi là một bệnh xã hội.  Ðiều trị lao phổi phaỉ đi song song với cải thiện xã hội. Ngày nào mà nghèo khổ, lạc hậu còn tồn tại thì ngày đó tiêu diệt bệnh lao phổi cũng vẫn chỉ là một giấc mơ.
